Documents Checklist

The documents you need may vary depending on your situation. You may contact us directly if you have questions. Please use the list below to prepare for your tax preparation appointment.

Personal Information
  • Your, your spouse’s and your dependent(s) full name, and date of birth.

  • Your, your spouse's and your adult dependent(s) identification card or driver's license, social security number card or Individual Tax Identification Number (ITIN).

  • Your, your spouse's and your dependents Identity Protection PIN, if one has been issued to you by the IRS.

  • Bank routing and account numbers to get your refund by direct deposit or pay taxes you owe, if you choose.

  • You, your spouse’s and your adult dependent(s) prior year's tax return.

Other Documents

  • Childcare or dependent records (including the provider's tax ID number) if applicable.

  • Retirement contributions, early withdrawal(s).

  • If you're a student or teacher, receipts for books, tuition and other education expenses.

  • Home mortgage and property tax records (if you itemize your deductions).

  • Donations to charity (if you itemize your deductions).

  • Health savings account or flexible spending account contributions.

  • Healthcare expenses (if you itemize. Special rules apply).

  • Retirement contributions.

  • If you're a student or teacher, receipts for books, tuition and other education expenses.

Income

Your employer, people or companies that paid you during the year are required to report the payments to the IRS. They must send a copy to you, and you should receive them by January or February.

  • W-2 Form(s).

  • Income of adult dependents and of other adults in your home.

  • 1099-G Form(s) for government payments such as unemployment benefits.

  • SSA-1099 Form(s) for Social Security benefits.

  • 1099-DIV Form(s) for dividends and distributions paid to you.

  • 1099-K Form(s) for payments from payment cards and online marketplaces.

  • 1099-INT Form(s) from banks and brokers showing interest you received.

  • 1099-NEC Form(s) for freelance and independent contractor work in the gig economy.

  • 1099-R Form(s) for retirement plan distributions or pensions or annuities.

  • 1095-A Form(s), Health Insurance Marketplace Statement, lets you reconcile advance payments or claims.

  • 1099-MISC Form(s) for other miscellaneous income.

Documents from Self-Employment
  • Income records, Forms 1099, Schedules K-1 to verify amounts not reported on 1099-MISC or 1099-NEC.

  • Records of all expenses — check registers or credit card statements, and receipts

  • Business-use asset information (cost, date placed in service, etc.) for depreciation

  • Office in home information, if applicable

  • Record of estimated tax payments made (Form 1040–ES)

  • Statements from banks, payment apps, card processors or online marketplaces.

  • Checks paid to you

  • Receipts and mileage logs for travel, gift and car expenses.

  • Estimated tax payments.

  • Other business income and expense records.
